Most City offices will be closed Nov. 23-24 for Thanksgiving. There will be no trash pickup on Thanksgiving Day. Residents whose scheduled pickup day is Thursday will receive service Friday, Nov. 24. Residents whose normal collection day is Friday will receive service Saturday, Nov. 25. This Acknowledgement and Right to Access form relates to utility services provided by and through the City of San Angelo. Those services are water, sewer, trash collection and stormwater. 

“ To ensure full security of your personal information, we request that you fax a copy of your drivers license and social security cards to (325) 653-8124.

Please be prepared to show both forms of identification when the processing of your application for service activation has been finalized.”

I agree the City may, in addition to its rights to terminate service to me after notice for nonpayment or service contract violation, disconnect utility service to the described location without any notice to me to mitigate a hazardous condition.

I hereby apply for services at the above address, which include water and/or sewer (depending upon the circumstances), trash and stormwater. I understand that a deposit is required with the amount being based on the size of the meter or past account credit history. I further understand that there will be a $20.00 non-refundable activation fee for each meter to cover maintenance and administrative costs related to opening an account. In addition, I (we) am aware that I (we) don’t need to be at the premises to connect service, but if the dial on the meter is moving, the technician will turn the water off at the meter as there may be leaks or open faucets. I understand that if I (we) request for City staff to return to the premises to turn the water on, a responsible person will need to be present and any attempt thereafter will include a $15.00 return trip fee.
