Pothole Repair Form

We encourage and appreciate the public's reporting of potholes, which assists us in their repair. Once submitted, this form will be sent to the City's Street & Bridge Division. Street crews will respond to reports in the order in which they were received and in as expeditious a fashion as possible.

Please tell us the location of the pothole by providing the nearest street address (ex. 123 Elm St.), the block in which the pothole is (ex. the 400 block of Main Street), or the nearest intersection (ex. Irving Street and West College Avenue).
